East Haldimand Hospice Inc.

East Haldimand Hospice has built a Clinical Group that works as an interdisciplinary team insuring that delivery of hospice palliative care is done so within professionally accepted standards of practice. This team has been instrumental in improving the quality of care provided to patients living with or dying from advanced illness. Success is evident in their collective ability to improve:

• Collaboration and cooperation among health care providers who support patients living with progressive life-limiting illness and their families

• The transfer of patient/ family information shared among health care providers

• Advocacy for patients living with progressive life-limiting illness and their families

• The knowledge level of health care providers in hospice palliative care

 

The next major project for East Haldimand Hospice Inc., as it pertains to the Clinical Group, is to collect statistical data that will validate the work they are doing within the Ministry of Health and Long Term Care “LHIN” system.  This Project ultimately improves the chances of accessing Ministry funding to continue to their work and meet their goals.

 

The Clinical Group meets bi-monthly and welcomes referrals from any medical professional who would like to increase the options and levels of support for their palliative patients (and families).
